Annual VPP Report Information and Guidelines

Information is now available with guidelines for the VPP Annual Report. Please go over these guidelines to insure compliance and meeting all requirements. Incomplete annual reports and annual reports that do not follow the correct format will not be accepted. A related message from Matt Gaines, Region VII VPP Manager, is posted below.

Dear Region VII VPP participants,

Please see the attached template for your 2014 Annual VPP Report. This report is due back to me no later than February 15, 2015. Please make sure that you use the correct three-year rate table that is under Section B. There is a link provided below for the 2013 BLS Injury/Illness statistics which you will compare your rates to. Also, please pay careful attention to Section D. You need to provide a written evaluation of all Elements and Sub-Elements that are listed in Section D. Please remember that a proper “evaluation” is not simply a description of how things are done. An evaluation will rate the overall effectiveness of each element and include recommendations for improvement if needed. Also, there is an attached example of an Annual Evaluation that was received last year and meets the requirements for what is necessary in an annual report.
Please note that you must follow the format attached below. Even if I have accepted your own format in years past, I am no longer allowed to do that.
Incomplete annual reports and annual reports that do not follow the correct format will not be accepted. We would prefer an electronic copy of your annual report when you submit.
